The rapid growth of the Internet of Medical Things (IoMT) has revolutionized healthcare by enabling continuous real-time patient monitoring using connected medical devices including wearable sensors, implantable monitors, and bedside diagnostic equipment. However, IoMT systems generate massive volumes of highly sensitive patient health data that must be securely stored, efficiently processed, and reliably transmitted while complying with stringent healthcare data protection regulations. Challenges including data security vulnerabilities, privacy breaches, scalability limitations, and inefficient data processing remain significant concerns that hinder widespread IoMT adoption. This paper proposes a Secure and Efficient Cloud-Centric IoMT Smart Healthcare System that integrates IoMT devices with cloud computing infrastructure to provide real-time vital sign monitoring, AES-256 encrypted data storage, intelligent analytics with automated anomaly detection, and role-based remote access for healthcare providers. The system implements a comprehensive security architecture combining AES-256 data encryption for confidentiality, JSON Web Token (JWT) authentication for device and user verification, and Role-Based Access Control (RBAC) for authorization management. Cloud-based analytics engines continuously monitor incoming vital signs against clinician-defined thresholds, generating automated alerts within 800 milliseconds for abnormal readings. Evaluation across 200 simulated IoMT patient devices demonstrates 99.2% data integrity with zero data corruption incidents, sub-second alert generation latency averaging 780ms, full HIPAA compliance through comprehensive audit logging, and linear scalability supporting concurrent monitoring of 200+ devices without performance degradation.
